Expect this form within 24 hours during the work week and on the following business day after a holiday or weekend. If you do not receive this form in the time frame indicated above, please call us so we can arrange to get another one faxed or sent to you. This Custom Sock Order Approval Form is your actual order that gets sent to the factory for production. Please proof it carefully for errors in quantities, sizes, style, colors, shipping address, etc. Without this signed form, we can not proceed with your order. Your order will be placed on hold or cancelled if this signed form is not returned within 3-4 days once we have sent it to you.

PRODUCTION TIME: Your order will be placed in the production schedule once this signed approval form is returned back to us.

Size Chart
Everyone varies in size. To get a better measurement, measure from the tip of the toe along the bottom of the foot and up the back of the leg. Then match that measurement to the tube length.
